- NEW: An outline of the proposed program/activity to be sponsored.
- W-9 Federal Tax Form: please print, sign and return.
- IRS Non-profit Status Form: view sample.
- Liability Waiver: complete, sign and return.
- Conflict of Interest Statement; complete, sign and return.
- NEW: If a permit is required (city, county, state, etc.), enclose copy of authorized document.
- NEW: Samples of previously conducted programs/activities, implemented by your organization. Enclose literature, curriculum, photos, class schedule (please no videotapes, CDs, DVDs)
