• NEW: If a permit is required (city, county, state, etc.), enclose copy of authorized document.
  
  
• NEW: Samples of previously conducted programs/activities, implemented by your organization.  Enclose  literature, curriculum, photos, class schedule (please no videotapes, CDs, DVDs)
